Patients History  

   - Thai woman 51 year old, BW = 196 kg, BMI = 87.1 kg/m2

   -  Can not walk for more than 4 years.

   -  Left Leg cellulitis in 1995, after that both left leg swelling

      for 11 years until now.

   - BP = 129/98, Pulse = 72

Diagnosis

   - Diagnosis from Ramathibodi Hospital: 1. Pulmonary Hypertension, 2. Hypothyroidism, 3. Morbid obesity

     4. Chronic renal failure, 5. Lymphatic obstruction 

   - Lymphedema (Gigantic)

Treatment

1. Reduction Phase (Admid in Hospital)

    - Day 1- 5:   Elastic bandaging (20-30 mmHg) and Supporter wrapping (30-40 mmHg)

                        for 6-8 hour a day (in patient car)

    - Day 5- 12: Elastic bandaging (20-30 mmHg) and Supporter wrapping (30-40 mmHg)

                        for 6-8 hour a day (in Hospital for Tropical Diseases)

    - Day 13-26: Twisting Tourniquet Technique (TTT) at Lymphedema Day Care Center 

    - Vegan diet (adjuvant therapy)

2. Maintenance Phase:  Apply  

     - Compression stocking (20-30 mmHg)

     - Self-bandage by use elastic bandage (20-40 mmHg)

     - Self-TTT at home during free time

     - Vegan diet (All 3 meals a day)
